Registration 2009 PALM BEACH JRS. 2010 Season Steps for Registration and Evaluations/Tryouts Register online with Palm Beach Jrs Click Here .  After you register you will be asked to print forms.  Please bring these completed forms along with $60 (evaluation/tryout fee) to your age appropriate evaluation/tryout.  If you have problems registering, please call Connie at 561-533-5655 or email her at ronvanderwey@bellsouth.net. Your 2009 membership is still good for August Tryouts.  You will need to renew after August 31st. for the 2010 club season.  If you are renewing your USAV membership, please select Login and renew your membership.  If you are a new USAV member, please select Register and join as a new member.  While registering, you will be asked for your club affiliation.  Please do not select Club 4 U –Palm Beach Jrs. unless you are sure that you want to play with us.  If you are not sure, please select Undecided for your club affiliation.  When you commit to a club, you need to log in and change your club affiliation one time to the club of your choice. If you have problems registering, please contact the USAV Florida Region office at 352-742-0080 or email michelle@usavfl.org.  Include player's name and birthdate. Attend the appropriate evaluation/tryout for your age group.  Please come dressed ready to play and bring the following paperwork:  -         Players information sheet -         Copy of USAV card – from USAV website (2009 cards are valid until Aug 31st.) -         USAV medical release (notary will be available) -         Copy of Birth Certificate -         Club Code of Conduct -         Parents Code of Ethics -         $60 Evaluation/Tryout Fee – cash or check made out to Palm Beach Jrs. In order to be selected for a team you must be registered online with Palm Beach Jrs., USAV, and pay the evaluation/tryout fee.  If you cannot attend your evaluation/tryout, please contact Terri at 561-586-1437 or kaisert@bellsouth.net  5.  Teams will be posted on our web site. You will then be able to accept or decline your position.  Once you accept, you will be able to print your Payment Contract.  Mail signed contract, credit card form, and registration payment ($400) to:  Palm Beach Jrs., 14 Redwood Ct., Boynton Beach, FL  33426. You may also pay online on our website under players and fax your forms to 561-533-5655 (no coverpage is needed).
